Select the File Type:

Begin by selecting what type of file you need to upload. Specific file types must be uploaded in order for your form to be submitted. If you're not sure which file yupe to upload, take a look at the Required Files section on your form to see what needs to be uploaded.

Select the File on Your Computer to Upload:
