What Is a Student Enrollment Record?
A student enrollment record is a record of a student's enrollment and attendance at a high school within a specified grading period. These enrollment records help to create a picture of the student's enrollment history and academic progress.
Where Do I Get the Information for an Enrollment Record?
The information to enter an enrollment record should be obtained from the student's transcript(s). Each school year and each grading period within that school year will have its own enrollment record.
Can I Skip Creating Enrollment Records in Lieu of Transcripts?
No. Every waiver form must have an updated student enrollment record for each semester/trimester for the student's high school career. Because transcripts vary from school to school, these enrollment records break down the important information and present a high level overview of the student's academic progress.
What Information Do I Enter?
School Year - The academic school year for this specific record.
Grade in School - What grade in school was the student for this school year?
Grading Period - The specific period for which the student attended.
Date Attended - The first date of the semester/trimester to the last date of the semester/trimester. Click the text field to bring up a calendar date picker to select a date in M/D/YYYY format.
School Attended - Which high school did the student attend during this period? If the school is an OSAA full member or associate member school, you can select it from the drop-down list. If the school is not listed, select "-- (other)" from the list and type in the other school's name in the Other School field.
Other School - If no school is selected from the drop-down list, you must provide a school name in this field.
# Credits Earned - The number of academic credits the student earned during this enrollment period.
# Classes Passed - The number of classes this student passed during this enrollment period.
